Chattopadhyay S, Pal A, Jana N, Maity D, Bandopadhyay S. Seasonal Variation of Delivery Rate: A Four Years Study in a Rural Tertiary Care Centre. |
Abstract: Objective: To determine if there are monthly variations in the rate of deliveries in Burdwan Medical College West Bengal, India. Materials and Methods: A retrospective descriptive study carried out in the Burdwan Medical College. We extracted the total number of deliveries from the hospital records for over four year period and analyzed the figures for the monthly variation in delivery rates. This was repeated for a standardized 30-day month and the findings noted. The extracted data then fed into a computer and analysis was performed, based on the grouping of the delivery rates per month. Results: The monthly distribution of total deliveries showed a slightly sinusoidal pattern with two peaks. A major peak spanning two months, August and September and another small peak in April and May. The figure also depicted the nadir number of deliveries to occur majorly in February and March and another in December. Conclusion: There is a seasonal variation in delivery rates in our study. Kumar D, Varun A, Shah U, Singh US. Health Status of Padia Workers in Slums of a Taluka Headquarters in Gujarat. |
Abstract: Background: ‘Padia’ is cup or plate made from paper. A cross-sectional study was carried out in 3 slum areas of Petlad town of Anand district in Gujarat state to describe the socio-economic and health status of the Padia workers using a pretested questionnaire. Results: 68.5% of the study participants were male. Only 27.6 % persons were educated upto higher secondary school and none beyond. There was no worker below 14 years of age.74 % work at their residence. Most of them spend about eight hours for the ‘Padia’ making work over and above their household job. Addictions were present in more than 1/3rd of workers. Musculoskeletal problem was commonest health problem. 18(12.1%) had experienced work place related injuries accidents. None of the workers used any protective devices. Conclusion: Padia workers have several health issues including occupation related health hazards. Need exists for a participatory occupational health programme for this working population. Abstract: Variations of the arteries and nerves are of clinical importance to the clinicians and surgeons in performing the surgeries. During the routine dissection for the undergraduates, variation in the branches of external carotid artery was noted in the right carotid triangle. The superior thyroid artery showed an upward loop from its origin and had a highly tortuous course, the lingual and facial arteries arouse from a common linguo-facial trunk and the ascending pharyngeal artery took origin from the Internal carotid artery. The ansa cervicalis showed absence of inferior root. The second and third cervical spinal nerves joined the superior root separately. Kashyap S. Primary Papillary Adenocarcinoma of Bilateral Fallopian Tubes. |
Abstract: Primary carcinoma of fallopian tube is one of the rarest malignancy of female genital tract accounting for about 1% of primary genital tract malignancies. Primary fallopian tube carcinoma clinically and histologically resemble primary ovarian cancers. The preoperative diagnosis of tubal carcinoma is rarely correct as the classical clinical triad is present in less than half of the cases and the usual ultrasound findings only suggest hydrosalpinx so, in most of the cases it is the pathologist who appreciates the tumor histologically. Because of the rarity of this tumor, a primary bilateral papillary adenocarcinoma of fallopian tubes in a perimenopausal female is presented highlighting its clinical, morphological and radiological features.

Manimaran D, Thulasi RR, Hemanathan G, Ishwarya S, Anuradha S. Eccrine Porocarcinoma Associated with an Eccrine Poroma. |
Abstract: Eccrine porocarcinoma is a rare carcinoma arising from the intraepidermal duct of eccrine sweat glands and constitute about 0.005% of epithelial cutaneous neoplasms. Eccrine porocarcinoma is commonly seen in lower extremities, followed by head, scalp, upper extremities, trunk and abdomen. It can arise denovo or from pre-existing eccrine poroma in 18% of cases. We present a 45 years old female patient who presented with painful hard warty growth of size 3x2 cm with ulceration and an adjacent nodule measuring 1x1 cm in the lateral aspect of dorsum of left foot for the past 3 years with rapid increase in size for 3 months duration. Microscopically larger and smaller nodules showed features of eccrine porocarcinoma and eccrine poroma respectively. Immunohistochemistry for pancytokeratin showed strong membrane positivity, Ki-67 showed 40% nuclear positivity and CD 34 negative. The lesion was diagnosed microscopically as eccrine porocarcinoma associated with pre-existing eccrine poroma.

Abstract: Obstructed hemivagina with ipsilateral renal agenesis (OHVIRA Syndrome) is a rare congenital anomaly. It mostly presents with severe dysmenorrhea and a palpable mass due to unilateral hematocolpos. Sometime it presents in unusual way with prolonged vaginal bleeding and profuse vaginal discharge. Here we report case of an 18 years old married female with OHVIRA syndrome presenting late with symptoms of recurrent pyocolpos following dilatation and evacuation for midtrimester miscarriage that was diagnosed on the basis of MRI and managed by vaginal septotomy.

Gopal G, Belavadi GB. Hutchinson-Gilford Progeria Syndrome. |
Abstract: Hutchinson-Gilford Progeria syndrome (HGPS) is a rare pediatric genetic syndrome associated with a characteristic aged appearance very early in life, generally leading to death in the second decade of life. Apart from premature aging, the other notable characteristics of children with HGPS include extreme short stature, prominent superficial veins, poor weight gain, alopecia, as well as various skeletal and cardiovascular pathologies associated with advanced age. The pattern of inheritance of HGPS is uncertain, though both autosomal dominant and autosomal recessive modes have been described. Recent genetic studies have demonstrated mutations in the LMNA gene in children with HGPS. In this article, we report a 16 years old girl who had the phenotypic features of HGPS and was later confirmed to have LMNA mutation by genetic analysis.

Abstract: Genu Recurvatum is a deformity of knee joint that tends to push it backwards by excessive extension in tibio-femoral joints. This poses a significant challenge because of technical difficulties and a high incidence of recurrence. This report describes a 63 years old male diagnosed as post-polio residual paralysis who showed excessive genu recurvatum of his left knee during long standing and walking. An Articulated Knee Hyperextension Orthosis (KAFO) was tried to check its effectiveness in terms of gait and energy expenditure.

Kamath SJ, Kamath GM, Roopashree. Bilateral Simultaneous Central Retinal Vein Occlusion Presenting as Initial Manifestation of Multiple Myeloma. |
Abstract: A fifty years old male with a medical history of diabetes mellitus presented with complaints of sudden painless bilateral vision loss since 2 weeks. Ophthalmic examination showed visual acuity in both eyes were counting finger 2 meters. Intraocular pressure and anterior segment examination findings in both eyes were normal. Funduscopy revealed bilateral central retinal vein occlusion (CRVO). Haematological and radiological investigations confirmed diagnosis of multiple myeloma and ocular manifestation in the form of bilateral simultaneous CRVO was the first presentation of multiple myeloma. This patient had a double hyper viscosity insult with severe diabetes mellitus and multiple myeloma. Since abnormal blood viscosity plays a role in the causation of bilateral CRVO, systemic investigation for underlying hyperviscosity syndromes including blood dyscrasias such as multiple myeloma is warranted.

Abstract: Primary squamous cell carcinoma of the endometrium is exceedingly rare. We report a case of 52 years old postmenopausal woman who presented with pelvic pain of four months duration. Gynecologic examination revealed a normal cervix. A possibility of pyometra was considered through pelvic ultrasound. Total abdominal hysterectomy was performed and histopathologically, it was diagnosed as a case of primary insitu squamous cell carcinoma of the endometrium.

Abstract: The medical gas cylinders have various safety features to prevent the administration of wrong gas to the patient. Most of the features have visual impact (colour, label, markings on the cylinder body) except pin index system on the cylinder valve, so error in this feature is difficult to analyze and is highly unpredictable. We had received one such cylinder in our institution where all other features (label, colour, marking) pointed to nitrous oxide except the pin index which resembled with that of oxygen.

Abstract: Antimicrobial stewardship (AMS) is one of the key strategies to prevent the emergence of antimicrobial resistance and decrease preventable healthcare-associated infections. But more than 50% of hospitals in India do not have an AMS program. The common barriers identified for implementation of AMS were lack of funding and human resource, lack of information technology, higher priorities, lack of awareness of administration and prescriber opposition. This paper analyses the possible solutions to overcome above barriers and promote effective implementation of AMS in India.
